MISHAPs: 04 NOV 41 A/C: PBY-3 Location: Gaton Lake, Canal Zone Strike: Nos BUNO: 0892 CAUSE: During the approach for the landing on which the damage occurred, the first pilot, while the plane was at 50', retarded the throttles to idle position to having the second pilot make a flat, power off landing. When it was noted that the plane was dropping faster than normal, the nose was raised slightly, causing the plane to strike the water in a nose high attitude, which resulted in a 5' to 6' bounce. Damage serious: General damage to hull between stations 5 and 6. Outside of hull: plates buckled aft of after strut base, all plats between stations 5 and 6 buckled. The engineer of the boat was pinned between that falling mast and the forward port gunwhale. Major overhaul. Pilot: Ens Robert B. Bernard, AV-N USNR & crew/Ok. MISHAPs: 14 AUG 42 A/C: PBY-3 Location: Manzanillo Bay, Canal Zone Strike: Yes BUNO: 0885 CAUSE: Plane taxied for take-off and ran into submerged rock pier. This pier was not bouyed or shown on any charts in the command and its presence was unknown to any of the pilots. Damage to hull resulted in plane sinking. Damage: Salt water submersion of plane and engines. Sever damage to hull. Torn and buckled bottom skin at portside sta. 1.33 to 5; starboard side sta. 4.5 to 5. Buckled bow around bombers window. MISHAPs: 24 SEP 42 A/C: PBY-3 Location: NAS Guantanamo Bay, Cuba Strike: Yes BUNO: 0884 Cause: Presence of numerous ship in normal take-off area, restricted pilots of choice of take-off course. Considering slight wind, the load of the plane, the pilot apparently underestimated the distance necessary to take-off. Flying speed was not attained by the time the opposite shore was reached. The throttles were cut and the plane was run up on the beach to a final stop with its nose in a sand dune. Damage: Bow caved in, hull twisted and skin badly wrinkled, tail cone broken off, port wing tip, float panel and struts damaged. keel broken and bent in several places.
